If you are a new client or coming in with a new patient, immediately after scheduling your appointment please fill out the New Client and New Patient forms found at the "Forms" link. Dr. Nichols' receipt of your forms a minimum of one week prior to your appointment is required so he may adequately prepare for your appointment. If we do not receive your forms at least a week before your appointment, you may be asked to reschedule.
Please arrive on time for your appointment as a late arrival shortens our available time with your pet. If you arrive more than ten minutes late, we may need to reschedule your appointment so as to not delay the appointments of patients scheduled after yours.
If you must reschedule or cancel your appointment, kindly provide us notice on a business day a minimum of 24 hours in advance of your scheduled appointment. This allows us to make your appointment time available to another patient. Unfortunately, if you do not contact us at least 24 hours on a business day in advance of your appointment or miss your appointment, the exam fee will be charged to your credit card. This policy is also clearly stated on your New Client form.
Patient Restraint Policy
For your protection, the protection of other patients and clients, and AADC staff, all dogs must be on a leash and properly controlled by their owner while in the reception area and exam room. We have slip leads available if you do not have a leash with you. All cats must be presented in an appropriate carrier.
For the safety of all patients in our care, all vaccinations must be up to date.
Payment is due in full at the time that services are rendered. For your convenience, we accept MasterCard, Visa, American Express, Discover, cash, and checks. AADC does not participate in the "Care Credit" program.
