Upon successful submission, you will immediately receive a confirmation message. If you do not see this confirmation on your screen, your form was not sent to us; thus, you need to review your answers for omissions then resubmit.
These forms must be completed and successfully submitted a minimum of one week prior to your appointment so Dr. Nichols may study the forms in conjunction with his meticulous review of your pet's medical records. If your forms are not received at least a week prior to your appointment, you may be required to reschedule to a later date.
